For owner-operators and 1099 drivers based around Dallas-Fort Worth, Houston, San Antonio, Laredo and El Paso, running I-35, I-10, I-20, I-45, I-30, I-40 and I-69.
Texas has more truck drivers than any other state, buys on HealthCare.gov, and has not expanded Medicaid. A 2025 review found no PPO plans at all on the Texas Marketplace, which is the core problem for OTR drivers here. Texas is also one of the three big domicile states for people who live on the road.

Laredo is the busiest land port in the country for U.S.-Mexico trade, and I-35 carries that freight north through San Antonio, Austin and Dallas-Fort Worth. Houston adds the port and the petrochemical industry, the Permian Basin adds oilfield hauling, and El Paso is a border crossing at the far end of I-10. A driver can spend a whole day crossing Texas and never leave it.
That size cuts both ways. A Texas plan built around one metro can leave a driver out of network in the next Texas city, before even counting runs to Oklahoma, Louisiana and beyond.
Before you buy, search the plan's directory for the hospitals your family would use at home, such as Memorial Hermann, Baylor Scott & White Health, Texas Health Resources, Houston Methodist and UT Southwestern, and for urgent care along I-35, I-10, I-20, I-45, I-30, I-40 and I-69.
The Robert Wood Johnson Foundation review of 2025 plans named Texas among large states with no PPOs on the Marketplace. Lineups and premiums vary sharply between rating areas, so what is true in Houston is not true in Lubbock. Texas has not expanded Medicaid, and some adults under the poverty line fall into a gap.
For healthy Texas drivers over the 400% line, a private plan on a nationwide PPO network is one of the few ways to get PPO-style reach in Texas. Private plan options vary by state, and we will show you exactly what is available at your Texas ZIP.
