The short version for North Carolina drivers

Quick answer

North Carolina drivers enroll on HealthCare.gov, and the state expanded Medicaid in December 2023, which closed the old coverage gap. If you checked your options before 2024 and found nothing, the answer has changed. North Carolina is also home to a major LTL carrier and a busy Charlotte distribution market.

Charlotte, the Piedmont Triad and the I-95 corridor

Charlotte is one of the Southeast's largest distribution markets, and the Piedmont Triad around Greensboro, where Old Dominion Freight Line is based in Thomasville, is LTL country. I-40 runs from the mountains to the coast, I-85 carries freight between Atlanta and Virginia, and I-95 runs the eastern side of the state. The Port of Wilmington adds container freight.

North Carolina drivers on I-85 and I-77 are in South Carolina and Virginia constantly, which a local network covers only for emergencies.

Before you buy, search the plan's directory for the hospitals your family would use at home, such as Atrium Health, Novant Health, Duke Health, UNC Health and ECU Health, and for urgent care along I-40, I-85, I-95, I-77 and I-26.

Expansion is new, and the lineup varies by county

Blue Cross Blue Shield of North Carolina participates statewide, and other carriers have expanded into more counties, so lineups vary by where you live. Since December 2023, adults up to about 138% of the poverty line generally qualify for Medicaid.

Above the 400% line, healthy North Carolina drivers usually compare a private plan on a nationwide PPO network with full-price Marketplace coverage.

About private plans: a nationwide network means in-network providers in many states, not that every service is covered. These plans are medically underwritten and not ACA-compliant: you answer health questions, a carrier can decline you or exclude a condition, and prescriptions and other benefits can be limited or capped.

Three things North Carolina drivers should know

  • If your income was low before 2024 and you were told there was nothing for you, check again; expansion changed the answer.
  • Charlotte drivers often live in South Carolina, or the reverse. You buy in the state where you live.
  • Eastern North Carolina drivers should look for ECU Health in the network.
