Start 60 days out

Quick answer

Losing employer coverage opens a 60-day special enrollment period that you can start up to 60 days before your last day. Use that time to compare COBRA, a full-price Marketplace plan and, if your household is healthy, a private plan on a nationwide PPO network. Replace group disability and life insurance before you leave, while you are still insurable on the same terms.

  1. Get your benefit end dates. Health, dental, disability and life can each end on different days.
  2. Apply for individual disability while still employed. Underwriting looks at your income and health; a gap in employment can complicate the income side.
  3. Price health coverage for the household. Private plans price each person's health; the Marketplace does not. A split, with different family members on different plans, is sometimes the best answer.
  4. Keep COBRA as a backstop. You have 60 days from the notice to elect it, and it is retroactive.
  5. Do not cancel anything until the new policies are issued.

When the family is home and you are not

Your spouse and kids will use doctors near home; you will need care wherever your assignments are. Check both: the network around your home ZIP for the family, and national reach for you. A nationwide PPO network can do both, if everyone qualifies medically. If someone in the family has a condition, they can stay on COBRA or move to a Marketplace plan while the healthy members go private.

The plain truth about private plans: the nationwide PPO plans we quote are private, medically underwritten plans, not Marketplace plans. A nationwide network means in-network doctors and hospitals in many states; it does not mean every service is covered. You answer health questions, and the carrier can decline you or exclude a condition. These plans are not ACA-compliant: depending on the plan, prescriptions, maternity, mental health and other benefits can be limited or left out, some benefits have dollar maximums, and out-of-network care costs more. We walk through exactly what a plan pays before you apply, and if a Marketplace plan fits you better, we tell you that too.

If you form an LLC or S corp

Many physicians run locums income through an entity. That does not by itself let you buy group coverage: a group plan needs at least one employee who is not an owner or an owner's spouse. S corporation owners also have specific rules for how premiums are reported and deducted. See S corp owner health insurance, and confirm the setup with your CPA.

Questions we hear most

How long do I have to get health insurance after leaving a hospital job?

You can pick a Marketplace plan up to 60 days before your employer coverage ends and up to 60 days after. COBRA has its own 60-day election window.

Should I get disability insurance before going locums?

It is usually easier to apply while you are still employed, with a clear income history and before any change in health. Group disability ends with the job.

Can my family stay on COBRA while I buy a private plan?

Yes. Each qualified beneficiary can make their own COBRA election, so family members can keep COBRA while others choose different coverage.
